STANDARD REQUIREMENTS
• 2 Valid Philippine Government Issued ID’s of Principal Buyer/Spouse (with 3 specimen signatures)
• Proof of Income
• Proof of Billing Address (electricity & water bills only)
• Marriage Certificate (if married)
• Latest CENOMAR (if single)
• BIR Verified Tax Identification Number (TIN)
Pag-IBIG Related Requirements
• Virtual Pag-IBIG Records of Membership Contribution
• 2 pcs. 1×1 ID picture (white background)
PROOF OF INCOME DOCUMENTS:
For Locally Employed:
• Latest three (3) months payslip
• Latest Certificate of Employment & Compensation (CEC)
• Latest Income Tax Return (ITR) or BIR 2316
• Latest Six (6) Months Bank Statements
For Overseas Filipino Worker (OFW):
• Latest Job/Employment Contract (duly certified by Department of Migrant Workers or Philippine Consular Office in the country where the buyer is working)
• Latest Salary Certificate/Certificate of Employment with Compensation (CEC)
• Latest three (3) months Payslips
• Latest six (6) months Bank Statements
• Latest six (6) months Proof of Remittance
• Special Power of Attorney (Notarized if executed in the Philippines or Authenticated by the Philippine Embassy or Consulate in the country where the buyer is working, if abroad)
For Self-Employed:
• DTI Registration (for single proprietorship)
• SEC Registration, Articles of Incorporation, By Laws, and Genera Information Sheet (GIS) (for partnership/corporation)
• Mayor’s/Business Permit
• ITR or BIR 1701 (certified true copy duly certified by BIR), Audited Financial Statements or AFS (with BIR received stamp), and Official Receipt of tax payment from bank (for the last 2 years)
• Sketch of Business Location (detailed with landmarks)
• Commission Voucher or Certification of Commission Received reflecting the issuer’s name and contact number & email (for the last 12 months)
• Bank Statements or Passbook (for the last 12 months) with authorization from the depositor to conduct bank verification (in case income is sourced from foreign remittances, pensions, etc.)
• Notarized Lease Contract and Proof of Ownership or Rights Over the Property Subject of Lease (if income is derived from rental payments)
• Certified True Copy of Transport Franchise issued by appropriate government agency and Valid Official Receipt (OR)/Certificate of Registration (LGU for tricycles, LTFRB for other Public Utility Vehicle or PUVs, MARINA for sea transportation) and Valid Official Receipt (OR)/Certificate of Registration (CR) of the Vehicle
• Certificate of Engagement issued by owner of business
• ITR or BIR 1701 (certified true copy duly certified by BIR) (for fees on professional services, commissions or services rendered for the last 12 months)
